Hospice Care

Hospice Care is not somewhere you go, it is an insurance benefit that allows individuals with life limiting illnesses, who no longer wish to seek curative treatment, to receive nursing care as well as a multidisciplinary team to address any needs of comfort they may have. Many benefits are included such as medications, equipment, personal care and a whole team of individuals working with you and your family to ensure quality of life.

Our hospice care focuses on quality of life, comfort and dignity for patients with a terminal illness whether their journey's end is months, weeks or days away.

The journey to the end of life is a family affair and our hospice team works with the entire family and support system to ensure that the journey is as peaceful and comfortable for the entire family.

Personalized Plan of Care

Collaborating closely with family members and caregivers, we craft a personalized plan of care for every patient receiving hospice or palliative care with Village Home Health and Hospice. We offer our patients an individualized care plan to manage symptoms, control pain and ease anxiety when they have chosen not to continue more aggressive treatments. Our shared goal is to ensure thoughtful and caring attention to every aspect of our patients' physical, emotional, social and spiritual selves.

Specialists in Hospice

Our experienced team of professionals includes Medical Director, Registered Nurses, Medical Social Workers, Chaplain, Hospice Aides, Consultant Pharmacist and volunteers. These dedicated specialists are available to patients and families 24 hours a day.

Hospice Eligibility Criteria

Have a terminally ill diagnosis with a physician certification and prognosis of 6 months or less if the disease runs its normal course. Willingness to discontinue curative treatments.
